LEOMINSTER – On Tuesday, Aug. 11, 2026, at approximately 11:13 a.m., Leominster Police Officer Kevin Costa was dispatched to Mechanic Street following a report of a violation of a harassment prevention order.

“Dispatch had confirmed that the order…was served and still active at this time,” Officer Costa said. “Upon arrival, I met with [the reporting party] (plaintiff), who stated that approximately 10 minutes prior to calling police, she was on her way to Ginny’s Food Pantry.”
The reporting party stated that as she was walking along the dirt path cut-through that connects Water Street to Mechanic Street, she observed Andrew Berg sitting on the side of the parking lot at 48 Mechanic Street as she approached the Mechanic Street side of the path.
According to police, Andrew W. Berg, 65, is homeless in Leominster, MA.
“[The reporting party] then stated that Andrew saw her and stated, ‘I will kill you, b*tch,’” Officer Costa said. “[She] stated that she was in fear, so she walked down Mechanic Street to provide space and wait for police.”
Officer Costa and other Leominster police officers then responded to the area described by the reporting party and found Berg still seated on the curb near the parking lot at 48 Mechanic Street.
“Andrew first stated that he did not remember the order or who [the reporting party] was,” Officer Costa said. “I advised Andrew that on July 12, 2026, I had served him with the extension of the order, which was active until July 6, 2026. Andrew then claimed that he knew about the order but has not seen her in person for over a year.”
Officer Costa said he then informed Berg that Box 2 of the harassment prevention order, which states the following, had been checked off:
- “You are ordered not to contact the Plaintiff. This means you cannot contact the plaintiff in person, by telephone, in writing, electronically, or otherwise, either directly or through someone else,” Officer Costa wrote. Adding, “You must also stay at least 10 yards from the plaintiff. You must not contact the plaintiff even if they seem to allow or ask you to contact them. The only exception is if some type of contact is allowed by the court as indicated below (there are no exceptions listed for this order).”
Officer Costa said Berg violated the harassment prevention order by contacting the reporting party in person and stating, “I will kill you, b*tch.”
Berg was then placed under arrest and transported to the Leominster Police Department for booking.
